Welcome to Connectedly Health, LLC (“Connectedly Health,” “we,” “our,” or “us”). These Terms govern your access to and use of our website, applications, and related services. By using the Services, you agree to these Terms, our Privacy Policy, and Cookie Policy.
Connectedly Health is an online directory and information platform connecting patients with healthcare professionals who operate outside traditional insurance-based systems. We do not provide medical advice, diagnosis, or treatment, and no provider-patient relationship is created through the platform.
Connectedly Health is not a healthcare provider, insurance company, or HIPAA business associate. HIPAA protections do not apply to information submitted to the Services, and providers are responsible for compliance within their own practices.
You are responsible for providing accurate information, protecting your account credentials, and all activity under your account. You must notify us promptly of unauthorized use.
Providers are responsible for the accuracy and completeness of their listings, credentials, pricing, services, and contact information. Verification supports directory trust but is not an endorsement or guarantee of care.
Patients choose providers independently. Connectedly Health is not a party to agreements between patients and providers and does not control or supervise healthcare delivery.
You retain rights to content you submit, but grant Connectedly Health a license to host and display it for the Services. Content must be accurate, lawful, and respectful; we may remove content that violates these Terms.
You may not misuse the Services, impersonate another person, submit fraudulent information, interfere with security, scrape the directory in a harmful way, or use the platform for unlawful purposes.
Provider subscriptions and other paid features are described at the time of purchase. Fees, renewals, cancellations, and refunds are governed by the applicable plan terms and billing information.
By using the Services, you agree to receive service-related messages. Marketing communications may be managed through the unsubscribe instructions provided with those messages.
The Services and their content, branding, software, and design are owned by Connectedly Health or its licensors and are protected by applicable intellectual-property laws.
The Services may link to or integrate with third-party websites and services. Connectedly Health is not responsible for their availability, content, privacy practices, or terms.
Some links are sponsored affiliate links. We may receive compensation when you use them at no additional cost to you. Affiliate relationships do not change the price or the independent terms offered by the partner.
The Services are provided on an “as is” and “as available” basis. We do not guarantee that listings, provider information, or the Services will always be complete, accurate, uninterrupted, or error-free.

We may modify, suspend, or discontinue any aspect of the Services and may update these Terms. Material changes will be reflected by updating the Effective Date.
To the maximum extent permitted by law, Connectedly Health is not liable for indirect, incidental, special, consequential, or punitive damages arising from use of the Services or interactions with providers and third parties.
You agree to defend, indemnify, and hold harmless Connectedly Health and its affiliates, officers, employees, and agents from claims arising from your use of the Services, your content, or your violation of these Terms.
Connectedly Health is not responsible for delay or failure caused by events beyond its reasonable control, including outages, natural disasters, or governmental action.
Using the Services constitutes consent to receive notices and agreements electronically. Electronic records satisfy any legal requirement that communications be in writing.
Disputes should first be addressed informally and, if unresolved, may be subject to individual binding arbitration under the applicable rules. The Terms are governed by applicable law and venue provisions described in the full agreement.
If a provision is unenforceable, it will be limited or removed as necessary. These Terms, together with the Privacy and Cookie Policies, are the entire agreement between you and Connectedly Health.
We may update these Terms by posting a revised version. Your continued use of the Services after changes are posted constitutes acceptance of the revised Terms.
